Kill a Harbringer (Horseman) for the first time. Have 7 or more red heart containers at one time. You can view your currently unlocked Achievements by going into Stats from the main menu, and then into Secrets.
Achievements are things that the player can unlock by finding new characters, items, and meeting certain milestones throughout the game.
